Djokovic is reportedly set for minor shoulder surgery which would rule him out of the remainder of the season.
World No 1 Djokovic retired from his US Open fourth-round clash with Stan Wawrinka because of the injury.
According to ABC, the Serbian will need a minor operation to ensure he is fighting fit for the 2019 campaign.
Djokovic has 2,600 ATP points to defend over the next two months because he won the Shanghai Masters and reached the final of the Paris Masters and ATP Tour Finals last year.
Nadal is currently 640 points behind Djokovic in the standings so would rise to the top if Djokovic misses just one of the remaining events.
The Spaniard had his own injury issues at the tail end of last year so is not defending any points himself heading into the final stretch.
Djokovic would remain 135 points above world No 3 Federer if his season was to draw to a premature close.
But the Swiss star will have the opportunity to better his 2018 semi-final appearances in Shanghai or Paris to end the year as world No 2.
